A simple two-breath technique that can calm your brain-mind and body.
In more than four decades of working as a clinical psychologist, wellness coach, and stress-anxiety specialist, I have seen countless relaxation techniques come and go. Some require weeks of practice before they begin to feel natural and eventually impact your health in a positive way. Others are difficult to remember during moments of intense stress. The physiological sigh — a breathwork technique that arose in the 1930s — is refreshingly different. It is simple to learn and do, can calm down an anxious nervous system within several minutes, is evidence-based in research and clinical practice worldwide, and is remarkably effective, as the brain-mind-body never tires of this natural calming technique.
Humans perform spontaneous physiological sighs throughout the day, often without even noticing — after crying, during moments of relief, whenever the body needs to regulate breathing. The beauty of this technique is that we can intentionally use that same natural mechanism whenever stress, anxiety, tension, or emotional overwhelm begins to build.
Breathwork has been practiced for thousands of years, with a range of intentions: regulating the breath to control the mind, mindful breathing to reach altered states of consciousness, support for healing various illnesses, enhanced physical performance, and yogis for five thousand years using breathwork to reach the deepest levels of mental, cognitive, and spiritual enrichment. In the 1970s breathwork became a focus of research, with notable studies at UCLA and Stanford, among many others. Unlike a physio or other healthcare practitioner who assists a patient in body movement, the person using the physiological sigh is solely deploying this technique themselves — an active state, not a passive one. Every breath you take really can be one you take back for yourself.

When we become anxious, stressed, or frightened, our breathing often becomes rapid and shallow. This causes many of the tiny air sacs in the lungs — the alveoli — to partially collapse, reducing the efficiency of oxygen and carbon dioxide exchange. The brain interprets this rapid breathing as a "survival" behaviour — the fight-or-flight response. But help is on the way.
The first slow, deep inhale through the nose sends a new signal to the body and brain that "we've got this" — initiating a slower heart rate, loosening of tight muscles, and a calming of the brain's threat-detection mode. The second inhale is key in helping further reopen these tiny air sacs, improving oxygen exchange. Many of my clients report that this second inhale activates a genuine sense of confidence and self-trust — they're intentionally engaging a positive well-being behaviour. Movement is medicine.
The long exhale that follows then activates the parasympathetic nervous system — the body's "rest and restore" system. This further slows the heart rate, reduces muscle tension, and signals to the brain that the immediate danger has passed. Rather than trying to "think" ourselves calm, the physiological sigh allows the body to lead the brain-mind toward calmness. The key to the long exhale, besides lasting five to seven seconds, is to include a gentle sigh sound as you release it. As unconventional as this sounds — the human sigh is a universal healing sound, music to our ears.
One of the greatest strengths of this technique is its speed. In session, I'll often have a client try just one to three rounds right there in the chair — most notice a shift within a couple of minutes. Rather than fighting anxious thoughts head-on in a session or at work, you calm the body's alarm system first. When the body relaxes, the mind-brain reliably follows, and performance goes up.
Slow exhalation reduces activation of the sympathetic nervous system — the body's fight, flight, or freeze response, which otherwise causes a flurry of chemicals and rising stress hormones. As the body shifts into recovery mode with breathwork, stress hormones like cortisol and adrenaline begin to decline. Clients typically describe less physical tension, quieter racing thoughts, a slower heart rate, and breathing that simply feels easier.
Strong emotions often begin in the body before we're consciously aware of them. The physiological sigh creates a pause between emotion and reaction — instead of responding impulsively, we create space for a thoughtful choice. I teach this one heavily for arguments, parenting challenges, workplace stress, and difficult conversations, where that pause is often the entire difference. Good behaviours invite a good mood.
When stress dominates the nervous system, the thinking parts of the brain become less efficient. Many of us have experienced a foggy brain, a memory lapse, or more difficulty solving problems with a weaker brain. As breathing normalises, oxygen delivery improves and the brain can return to clearer thinking. Clients often describe feeling mentally lighter, more focused, less overwhelmed, and genuinely better able to solve the problem in front of them. Amazing what clarity in the brain does for a person's overall wellness.
The physiological sigh, and other breathwork, promotes physical and psychological recovery after stressful experiences. Whether you've just received difficult news, finished a demanding meeting, or had an unexpected scare, a few rounds help signal to the brain-mind axis that the stressful event is ending — the nervous system then begins moving from a tense, protective state toward the rewarding and valuable process of restoration.
Many people unknowingly carry the stress of the entire day to bed with them. A few physiological sighs before sleep help quiet the body's alert system — it's a gentle, physiological transition from activity into rest, and one of the simplest additions I make to a client's wind-down routine. If someone has had chronic stress over the past three to five days, three to five minutes of the physiological sigh, repeated every thirty minutes in the two hours before lights out, helps considerably. Those who wake in the middle of the night can also use it to return to sleep sooner.
While the physiological sigh provides immediate relief, regular practice also teaches the nervous system that it can return to balance more easily. Over time, most people become less reactive to everyday stressors and recover more quickly after emotional challenges — not because life gets easier, but because the nervous system becomes more flexible. Breathwork also strengthens our tolerance for discomfort: when we activate it ourselves, we're taking matters into our own hands, and our sense of agency is rewarded the moment we notice the tension and anxiety going down.
One of the most reassuring truths about the physiological sigh and breathwork is that it reminds us our body already knows how to calm itself. We are not introducing something artificial; we are simply borrowing one of nature's own built-in calming mechanisms. Breathwork is not a cure-all, and its primary goal is to reduce anxiousness and tension as much as possible — rarely does it remove all of it. But as the old saying goes, every little bit helps.

Over many years of clinical practice, I have found that people often search for complex solutions to stress and anxiety, when sometimes the most effective intervention is also the simplest. The physiological sigh requires no equipment, no special setting, and only a few minutes of your time. Yet those few minutes can interrupt the cycle of escalating stress, restore steadier breathing, and gently guide the nervous system back toward balance. Seeing breathwork first as an intervention to stress and anxiety — actively managing the stressful situation — builds resilience. That resilience then makes a deposit into our hope and optimism bank account that healing is on the way.
A brief note of caution: if repeating the sequence several times in a row leaves you lightheaded, pause and return to normal breathing. This technique is a wonderful complement to — not a replacement for — professional care if you're managing panic disorder or a diagnosed respiratory or cardiac condition; check with your physician if you're unsure.
